Responsibilities:
Conduct comprehensive assessments of residents' health status and develop individualized care plans in collaboration with the interdisciplinary team.Administer medications, perform treatments, and implement nursing interventions according to physician orders and established protocols.Monitor residents' vital signs, laboratory values, and diagnostic test results, and communicate findings to the healthcare team.Provide education and support to residents and their families regarding health conditions, treatment plans, and self-care techniques.Supervise and delegate tasks to licensed practical nurses (LPNs), certified nursing assistants (CNAs), and other healthcare personnel as appropriate.Document nursing assessments, interventions, and outcomes accurately and timely in residents' medical records.
Qualifications:
Valid Registered Nurse (RN) license in PA.Previous experience in long-term care or rehabilitation settings preferred.Strong clinical skills and knowledge of nursing principles and practices.Compassionate and patient-centered approach to care.Excellent communication and interpersonal skills.Ability to work effectively as part of a multidisciplinary team.Basic computer skills for documentation purposes.
